Wade, who missed the previous four games with a sprained right wrist, shot 9-of-17 from the field and 15-of-17 from the foul line. He also hit four big foul shots in the final 22.8 seconds, as the Heat snapped a five-game skid.
Jason Kapono 19 points, while Udonis Haslem contributed 12 points and 10 rebounds for Miami, which rallied from a 16-point deficit to beat Portland for a fourth straight time.
Brandon Roy scored 18 points, but missed a potential tying three-pointer at the buzzer, as the Trail Blazers lost for the fifth time in six contests. Zach Randolph ended with 15 points and LaMarcus Aldridge a career-best 14 for Portland.
Randolph’s bank shot boosted Portland to an 86-85 edge with 2:19 left, but Alzonzo Mourning hit a hook shot to give the Heat the lead for good at 87-86 with 1:53 remaining.
Randolph then missed a tough shot from the left baseline, and Wade jammed at the other end.
Aldridge’s left baseline jumper had the Blazers within 89-88 with 23.4 seconds left, but Wade hit four free throws, sandwiched around a layup from Ime Udoka, to give the Heat a 93-90 edge with 7.9 seconds remaining.
Roy then tried a three under heavy pressure from the right wing, but the shot hit off the backboard as time expired.
Dorell Wright tallied 12 points for the Heat.
Martell Webster scored 12 for Portland, while teammate Jarrett Jack had a career-best 11 assists.
The Blazers led 30-22 after one quarter and the advantage stood at eight (60-52) at halftime. However, the Blazers extended the margin to 70-54 with 7:07 left in the third after Jack’s layup.
Miami was within 77-69 going into the fourth and scored the first six of the final period to keep it close until the closing moments. Seven straight Heat points, capped by a Kapono jumper, moved Miami to an 82-81 lead.

The Heat shot 46.4 percent form the field, compared to 43.8 percent for Portland...Miami went 25-of-30 at the foul line, compared to 5-of-7 for the Blazers.
